Schedule Appointment

Medium Hardware Assurance Certificate issuances require an in-person appointment. To schedule an appointment, please fill out the request form below.

Once your appointment has been scheduled, you will receive a confirmation e-mail with instructions on next steps.

You will be required to bring in that e-mail on the date/time of your appointment.

If you do not have your account created/registered and all of the required documentation at the time of your appointment you will be turned away and asked to re-schedule.

If you cannot find an appointment slot that meets your needs due to an emergency or inspection, please send an email to to schedule an appointment outside of the slots provided.


Please remember, you will need to bring the necessary forms of identification to obtain a certificate.
