Medium Hardware Assurance Certificate issuances require an in-person appointment. To schedule an appointment, please fill out the request form below.
Once your appointment has been scheduled, you will receive a confirmation e-mail with instructions on next steps.
You will be required to bring in that e-mail on the date/time of your appointment.
If you do not have your account created/registered and all of the required documentation at the time of your appointment you will be turned away and asked to re-schedule.
If you cannot find an appointment slot that meets your needs due to an emergency or inspection, please send an email to wcsc.helpdesk@widepoint.com to schedule an appointment outside of the slots provided.
Please remember, you will need to bring the necessary forms of identification to obtain a certificate.
